My optin form submitting without even entering name and email in IE/Safari/chrome

Hi
I amusing Office Autopilot autoresponder and I added my opt i form code to my squeezepage template. Its working perfect in Firefox but not in IE/Safari/Chrome. In these browsers it can be submitted without even entering required field "name" and "Email" in the box. What is the problem, if someone can helpme out please. Here is my form center code from my squeeze page template:

Hi,
I think its CSS issue the code , frankly i feel you need to hire an expert for this, I suggest hiring someone from microjob sites for few bucks only. If you have any hosting related issue, or need further info I be Happy to Answer :)
